The post holder will be an experienced clinician within their professional boundaries; assess, diagnose and manage patients with a range of acute, non-acute and chronic medical problems. Show effective clinical decision making and undertake regular job planning with the clinical lead.
The post holder will work collaboratively with all multi-disciplinary general practice team to meet the needs of patients. Post holder will use their individual skills and experience in order to make a positive contribution to the organisation and development of a ‘one team’ approach to provide high quality patient services within and across the primary care domain.
Key requirements:
Must have experience with long term conditions and have good scope of general practice
o Attention to detail
o Able to work efficiently, while adapting to different environments
o Industry body (GMC / NMC / HCPC] registered
o Ability to make decisions in high-pressure situations
o Triage Skills
o Registered Prescriber
o System one – Desirable (Training will be provided)
o 2 – 6 Sessions
