The Health Services Safety Investigations Body (HSSIB) – the independent and no blame investigator of patient safety incidents – has recently launched work considering risks faced by people who self-administer insulin in community settings, with a focus on particularly vulnerable and disadvantaged groups.
For this current piece of work, the HSSIB team are keen to identify example cases where patients have faced safety issues because of difficulties/barriers to supporting their self-administration of insulin (for any type of diabetes) where they have co-existing mental health conditions and/or cognitive difficulties.

The HSSIB has previously done work on online consultations which reflected many of the points Londonwide LMCs hears raised by GPs. They also have an investigation into electronic communications on patient discharge from acute hospitals.
