About Us: Clifton Rise Family Practice serves a vibrant community of approximately 4500 + registered patients. Our friendly team is committed to excellence in patient care and professional development. We boast an established multi-disciplinary practice team, which includes a GP Partner, Salaried GPs, Practice Nurse, Physician Associate, First Contact Physiotherapist, Mental Health Worker, Social Prescriber, Health and Well-being Coach, and Care Co-ordinator.
