This letter is intended as a broad template that can be adapted to cover a range of applications where a patient may ask their GP to write a letter or fill in a form in support of an application, request or appeal for access to a particular benefit or service.
In the majority of cases the central and local government bodies administering such requests have a policy of solely considering what health conditions a patient has, as indicated by the coded record they should already have access to, and will not take into consideration any arguments made in an additional letter by their GP. Such letters are also not activity that is funded via the GMS contract.
